To ensure the highest level of personal attention and service, each new registration starts with a parent & child visit to ABC Kidz Kare.  After completing the form below, you will be contacted by one of our staff to setup a personal appointment to visit and tour the center, meet our director and staff, ask specific questions and begin completing your child's enrollment paperwork.

For each child registered, parents are required to complete a pre-enrollment packet of information.  This packet is to be returned to the Center’s office prior to the child’s first day of attendance. 

The pre-enrollment packet contains the following items: 

Emergency Contact/Parental Consent Form (Form CY-867)

Health Assessment Form (Form CY-51)

Financial Agreement Form (Form CY-321)

Release of Information Log (If necessary)

Signed statement verifying receipt of parent handbook

 

This packet will be provided to you during your initial visit.
